Needles can be placed anywhere on the body, with the exception of inside the mouth and around the genital area. A single needle may be all that’s required but treatments typically involve multiple needles — 8 to 10 on average. Needles penetrate to depths of from a fraction of an inch, up to 5 or 6 inches, according to their placement on the body. Physician practitioners leave them in place anywhere from a few seconds to 30 minutes, while Molony says they are sometimes left in for up to an hour.

Does acupuncture hurt? Watts says if it does, you need to find another practitioner. You may feel something for a split second, which Watts describes as radiation or perhaps an itchy sensation. Molony said, “Most of my patients are incredibly surprised at the lack of pain.”

Costs
Traditional acupuncture treatments average $45 to $85 per visit. Another testament to its mainstream acceptance is the fact that insurance companies now reimburse for this treatment. According to John Weeks, publisher/editor of The Integrator for the Business of Alternative Medicine, a publication that tracks business trends in integrative medicine, about 30 percent of managed care organizations and employers offer some form of coverage for acupuncture. He notes it may be severely limited coverage — for example, paying only for a medical doctor to treat a specific condition. Also, Weeks says, HMOs are increasingly willing to contract with networks of alternative health practitioners and provide members with discounts on cash services. Medicare and Medicaid do not currently reimburse for any acupuncture procedures, but pending legislation in Congress is attempting to change that.

Safety Concerns
Frank says the practice of acupuncture is safe in well-trained hands. All states require knowledge and practice of clean-needle techniques.

Molony notes that virtually all Eastern practitioners use disposable needles, so the risk of disease transmission is eliminated. While incorrect needle placement can cause serious damage, such complications are extremely rare.

Watts, who is the president of the Academy of Oriental Medicine in Austin, Texas says the reason acupuncture continues to grow in popularity as a complementary therapy is simple: “Because it’s safe and it works. The Chinese are very smart people. If it didn’t work, they would have thrown it out thousands of years ago.”

In 1997, a consensus panel convened by the National Institutes of Health concluded that there is clear evidence that needle acupuncture is effective for postoperative and chemotherapy nausea and vomiting, nausea of pregnancy, and postoperative dental pain. The panel also concluded that it holds promise for treating addiction, stroke rehabilitation, headache, menstrual cramps, tennis elbow, fibromyalgia (general muscle pain), low back pain, carpal tunnel syndrome and asthma.

In the August 2011 issue of the Archives of Internal Medicine, researchers reported that cocaine dependent patients who received a course of auricular acupuncture were more likely to be free of cocaine during treatment than those not receiving acupuncture.

The NIH has ongoing clinical trials investigating the effectiveness of acupuncture to treat a number of conditions, including osteoarthritis, craniofacial disorder (TMJ) and neurological disorders.

The World Health Organization lists 41 diseases that respond well to acupuncture. These run the gamut from acute sinusitis and cataract to gingivitis and neurogenic bladder dysfunction.

Stuart Watts, O.M.D, L.Ac., believes history alone says great deal. “A quarter of the world’s population has been treated with Oriental medicine for the last 2,000 years. That’s a lot of case histories.”

Training and Licensure
There are 45 schools in the United States offering an accredited 2,400 to 3,200 hour curriculum in acupuncture and Oriental medicine. Watts, who is considered a pioneer in establishing training and credentialing standards for acupuncturists in the United States, says this includes at least 1,000 hours of closely supervised hands-on clinical training. Nonphysician practitioners must be licensed or registered in 40 states and the District of Columbia. Requirements for certification in most states include completion of training from an accredited school and passing the National Certification Commission for Acupuncture and Oriental Medicine (NCCAOM) exam. Maryland and California have slightly different requirements.

In most states, physicians can practice acupuncture under their medical license, with or without specific training. In eight states, a separate license is required. Physician training involves a 200-hour course, including 100 hours of clinical practice, according to Jim Dowden, executive administrator of the American Academy of Medical Acupuncturists. The American Board of Medical Acupuncture also certifies physicians in the practice.

Physician training is based on a European interpretation of Asian medicine, known as “energetics” and the neuroanatomical model, which uses the human system of anatomy, muscles and nerves as its foundation.

Frank explains that Western and Eastern training differ in the organizing principles for diagnosis and treatment.

Visiting an Acupuncturist
A visit to a traditional acupuncturist will last from 60 to 90 minutes. The practitioner asks questions and hones in not only on your words but the tone of your voice, your body language, facial color, fingernails, even how you smell. Your tongue will be examined, as it’s believed to be a mirror for the entire body. Your pulse will be taken and the practitioner will touch various points throughout the body.

In high school, Bryan Frank lived for several years with a painful chronic back problem that conventional medicine couldn’t touch. He turned to an acupuncturist, and the problem vanished within three visits. That was 28 years ago, and today, Bryan Frank, M.D., says that “dramatic example” is one of the reasons he uses acupuncture in his Dallas, Texas-based anesthesiology and pain medicine practice.

In the eight years Frank has been using acupuncture, he has concluded that it’s really more effective than Western medicine in treating both acute and chronic musculoskeletal disorders. “It’s the most important thing I have in my doctor’s black bag for sports medicine, for example.”

Frank, who is president of the American Academy of Medical Acupuncture, says he has also had great success using it to treat anxiety and depression, asthma, and stomach and gut problems. “Combining Western and Eastern approaches generally allows me to treat patients more quickly and effectively with less procedures and medicines and less risks associated with the use of conventional therapies,” Frank said.

Marshall H. Sager, D.O., president elect of the American Academy of Medical Acupuncture, began specializing in acupuncture more than 10 years ago, after 25 years in a suburban Philadelphia family medical practice. According to Sager, who is also chairman of the American Board of

Medical Acupuncture, Eastern medicine focuses on the person with the problem while Western medicine concentrates only on the problem a person is experiencing. While 75 percent of his patients come for pain relief, Sager says medical acupuncture has been very useful in treating patients suffering from colitis, infertility, arthritis, allergies and irritable bowel syndrome. For Sager, it is the combination of Eastern and Western paradigms that offers his patients “the best of both worlds” of healthcare.

“Of course you’re not going to use acupuncture as a primary therapy to treat cancer,” said Sager. “That is best handled by Western medicine. But having a physician who practices medical acupuncture as part of your healing team can be invaluable. I frequently use medical acupuncture adjunctively to ease the discomfort of chemotherapy and radiation, and to build the immune system. The same would be true for a heart attack or broken bone.”

What Is Acupuncture?
It’s estimated that 10 million to 12 million Americans visit some 12,000 non-physician acupuncturists and nearly 2,000 physician acupuncturists each year. Other clinicians, including chiropractors, podiatrists and even dentists are also practitioners of this form of traditional Chinese medicine. So what is it?

How and why acupuncture works is not precisely known, and is the subject of some debate in the medical community. It’s based on the belief that energy flows through the human body within a network known as meridians. David Molony, Lisc. Ac., Dipl.Ac., C.H., explains that meridians are somewhat like feedback loops, and serve as a mechanism for the body to communicate with itself. It is believed that energy focuses — or pools — along these pathways in 364 locations or points. Acupuncture involves the insertion of extremely thin, flexible needles into these points, with the objective to speed up or slow the flow of energy.

There are several types of acupuncture. Needling involves the insertion of needles alone. When electricity is used to stimulate acupuncture points, this is known as electroacupuncture. Moxibustion uses a Chinese herb to warm the acupuncture point, with or without needles in place.

Cupping is the ancient form of placing suction cups over various parts of the body. Low-level laser light is also used to stimulate energy pools that are believed to exist. Acupressure involves applying pressure without the use of needles.

Various microsystems exist within medical acupuncture, wherein the entire body can be treated by stimulating points within a small area of the body, such as the ear, scalp, face, hand and foot. Auricular acupuncture focuses on the ear and is typically the method used for treating addictions. Korean hand acupuncture employs various tools other than needles to stimulate points, and is favored by some physicians in working with younger patients, who find it less threatening than needles.

Physical activity

Regular physical activity increases plasma volume. An increase in plasma volume means that the blood is more dilute, or “thinner,” with a lower hematocrit (red cell count) and fibrinogen level and, consequently, a reduced risk of clotting. Several studies have shown that vigorous exercise also reduces platelet aggregability and enhances fibrinolytic activity. These effects may help to explain why active people are at lower risk for heart disease and stroke. Like most of the beneficial effects of exercise, these changes are short-lived, disappearing after two or three days. Regular, lifelong physical activity is the type of exercise that reduces clotting risk. A sedentary lifestyle, on the other hand, leads to increased clotting risk; blood thickens as plasma volume decreases. Sedentary people have stickier blood platelets, which in conjunction with higher levels of fibrinogen, are more likely to form blood clots.

An application of these observations is seen in the advice given to people prone to blood clots: Avoid sitting for extended periods, such as long airplane flights or car rides. Inactivity creates sluggish circulation in the arms and legs, and gives clots a better chance to form.

Stress

In the days of cave men and women, fighting and fleeing were often associated with bleeding, so it makes sense that enhanced clotting speed is part of the fight-or-flight response, even though clotting is not so useful nowadays. Prolonged mental stress impairs fibrinolysis by decreasing the activity of an enzyme called tissue plasminogen activator (tPA), which helps break down fibrinogen.

Heart-healthy diet: Reducing atherogenesis and thrombogenesis

People with high serum cholesterol levels exhibit disturbances in coagulation, fibrinolysis and platelet behavior. Lowering blood lipid levels by diet or drug therapy seems to reverse these disturbances, and may be one mechanism whereby a heart-healthy lifestyle reduces heart disease risk, in addition to its effects on atherogenesis. An interesting study from Denmark found that volunteers who stuck to a low-fat, high-fiber diet showed increased fibrinolytic activity and, thus, a reduced risk of blood clot formation.

A moderate alcohol intake (one to two drinks per day) has been associated with a reduced heart disease risk. This risk reduction may be due in part to the increase in fibrinolysis observed in moderate drinkers.

What would reduce the risk of heart complications, heart attack, stroke in men who suffer from diabetes it is took simvastatin online (C25H38O5), that prevents the organism’s synthesis of cholesterol.

Many people fear cholesterol, that evil substance that silently accumulates along artery walls, year after year, until it eventually kills its victim by blocking blood flow to an important organ such as the heart or brain. But cholesterol is not the only villain in this atherosclerosis melodrama. While cholesterol contributes to the formation of arterial plaque, the antagonist delivering the final blow is often a blood clot that forms in a blood vessel and subsequently blocks a narrowed artery, cutting off circulation to the tissues downstream.

Why do blood clots form?

Fibrin forms blood clots in conjunction with disc-shaped blood cells called platelets. When platelets come into contact with damaged blood vessels, they enlarge and become sticky, and begin to adhere to collagen fibers in the damaged area. They produce substances that activate other platelets, and cause them to enlarge and become sticky as well. The activated platelets clump together, a process known as platelet aggregation. This accumulation of platelets forms a mass that turns into a clot when it becomes bound together with fibrin threads.

Can this get any more complicated? Yes. Even as the blood clot is forming, the body prepares a check-and-balance system to keep clotting under control. Plasmin is a “helper” in the atherosclerosis picture, because it can dissolve small blood clots in blood vessels before they do any harm. Fibrinolysis refers to the dissolution of a blood clot due to the inactivation of fibrin.

If you have gotten this far in our biochemistry lecture, you can see why it is important to discourage clotting and encourage fibrinolysis.

Atherosclerosis, hypertension and risk of clotting in blood vessels

Health professionals have been promoting a “heart-healthy” lifestyle for years to prevent, or at least significantly slow, the process of atherosclerosis by preventing hypertension, obesity, high blood lipid levels and NIDDM (non-insulin-dependent diabetes mellitus). The same factors that are part of a heart-healthy lifestyle have an added benefit: Most of them also reduce the tendency of the blood to clot when it encounters injured endothelial tissue.

Atherosclerosis increases one’s risk of clot formation because arterial plaque activates platelets which then adhere to the injured blood vessel wall, initiating platelet aggregation and the formation of thrombin. Simply preventing atherosclerosis through a heart-healthy lifestyle and/or drug therapy helps to prevent clotting. A heart-healthy lifestyle also helps to prevent clotting by preventing hypertension. Even borderline hypertension is associated with increased platelet aggregation and diminished fibrinolytic activity.

Smoking

If you need another reason to quit smoking, here it is. Smoking increases blood fibrinogen levels. Increased fibrinogen levels are associated with increased clotting risk. High fibrinogen levels increase platelet aggregation and fibrin deposition, and contribute to both clotting and plaque deposition.

What drop the risk of heart attack, heart complicationsandstroke in people with coronary heart diseaseordiabetes or other risk factors it is oftenused simvastatin (C25H38O5), which prevents the organism’s production of cholesterol. Simvastatin it’s a safe hypolipidemic remedy.

Imagine this: Sitting down at a table, complete with china, linen tablecloth, and all family members sharing a wholesome meal and pleasant dinner conversation.

If this seems like an episode of “Leave it To Beaver” to you, you’re not alone. Fewer than half of American families sit down together to eat each night. And 17 percent never eat together — or eat together only occasionally. The crush of family activities, combined with a hurried lifestyle and dual-income families has made eating together challenging at the very least.

What’s the big deal about eating together? According to a study recently published in the Archives of Family Medicine, kids 9 to 14 years old who most often ate dinner with their families had the most nutritious diets. This means their diets were closer to the recommended “five-a-day” servings of fruits and vegetables and were lower in fat and sugar. And a study from Harvard University found that preschool children who were exposed to frequent dinner-table conversation developed wider vocabularies and better reading skills than those exposed to less-frequent mealtime conversation.

With such dramatic benefits associated with eating together, why is it so difficult? Dinnertime may be a tough time to coordinate everyone’s schedules, given multiple family commitments. And often, the time it takes to make a family meal is just too much to fit into an already loaded schedule. As a result, many families choose to cruise through the local fast-food drive-through and eat on the run. Americans eat a whopping 27 percent of their meals away from home, a dramatic increase from the 1950s.

How do some families manage to squeeze in time together? One common factor is that successful families minimize food preparation time, opting instead to spend this time at the kitchen table together.

Others choose less-traditional dinner times, choosing to dine together at 5 p.m. or 7:30 p.m. Few, if any, worry that the table linens are just right or that the table is set perfectly. Like any successful endeavor, a little preparation saves time later, when nerves are frazzled and tempers are short.

Good nutrition is an important part of staying healthy, so your members may be interested to know that the federal government released its new dietary guidelines in May. The guidelines emphasize fitness, weight control and food safety, in addition to a healthy diet. Says U.S. Secretary of Health and Human Services Donna E. Shalala, “These new guidelines emphasize the importance of exercise in maintaining proper weight – especially since 55 percent of the adult population is overweight.”

The government did not modify the current food pyramid, and still calls for a “common sense approach” to diet and exercise. The dietary guidelines call for a diet that is low in saturated fat and cholesterol and, as in previous editions, recommends a daily fat intake that represents no more than 30 percent of total calories. The guidelines also recommend that foods be prepared with less salt and that adults who drink alcohol do so in moderation. The breakdown of portions include the following: 6 to 11 portions from the bread, cereal, rice and pasta group; 2 to 4 portions from the fruit group; 2 to 3 servings from the meat, poultry, fish, beans and eggs group; 2 to 3 portions from the dairy group; and 3 to 5 portions from the vegetable group. Guidelines also advise using oils, fats and sweets sparingly.

Emphasizing the need for food safety, U.S. Surgeon General Dr. David Satcher said, “We are concerned with eliminating the risk of foodborne diseases like hepatitis A in strawberries or E. coli in ground beef, which we have seen in the past since more and more of our food supply is a global food supply.”

Now people can order pain medication without prescription. We may recommend you the largest range of safe and effective medicines. All of this remedies only the highest class and have detailed description, indications to application.

Imagine being at a health club, prepared to get a good weight-training workout. You climb on the leg extension machine where the pin is set to lift about 80 pounds. Slowly and methodically you perform the movement. After 10 to 15 repetitions, you cannot lift another. You wait a minute, then perform a second set in the same fashion.

Near you, on the overhead press machine, a guy is jerking the weight stack up and down as fast as he can. You start to wonder. “Should I be lifting super-fast like he is? Is that guy getting more benefit from doing rapid repetitions?”

Often referred to as “explosive lifting,” performing repetitions in a high-velocity manner that is ballistic in nature has been favored by many competitive weightlifters and others who mistakenly believe the practice will enhance a person’s speed, power and explosiveness as an athlete.

Lifting weights in a rapid, explosive fashion is not recommended for two reasons. First, it introduces momentum into the movement, which makes the exercise less productive and less efficient.

After the initial burst of motion, the muscles throughout the remaining range of motion encounter little or no resistance. The weight, in effect, is moving under its own power due to the building momentum and inertia.

Explosive lifting can also cause injuries. Using momentum to lift a weight increases the internal forces placed on a joint; the faster a weight is lifted, the greater these forces are. Damage occurs when the forces exceed the structural limits of a muscle, bone or connective tissue. If you choose to explode with a weight, therefore, you are increasing your risk of incurring an injury.

The Globe said figures in Lilly internal documents showed that in early clinical trials, 1 in 100 previously nonsuicidal patients who took Prozac became anxious and agitated, and either attempted or committed suicide during the studies.

Some of the previously unknown information that Eli Lilly possessed about the drug has come to light during civil trials in which the company was sued for wrongful deaths following suicides, or during criminal trials in which defense lawyers argued that Prozac caused a defendant’s violent and homicidal behavior.

Maris has been an expert witness in a number of trials, including one in Hawaii last year in which Healy also testified. William D. Forsyth Sr., a retired businessman, stabbed his wife 17 times and then stabbed himself to death. He had been taking Prozac for 10 days. His family brought a wrongful death suit against Lilly, claiming that the company knew Prozac could make some people violent or suicidal, and failed to notify physicians about it.

The plaintiffs wanted $1 million in damages and the addition of a warning statement to the Prozac label. A jury found for the defendant, Lilly. So far, Lilly has won every Prozac-related civil suit against it, more than 100 and possibly 200. The number is difficult to verify because most did not go to trial. Some — Lilly says only a few — were settled out of court to avoid the expense of defending them.

In criminal cases up to now, prosecutors, frequently with the help of expert witnesses suggested by the company, have been able to obtain convictions despite defense claims that the drug had turned a previously stable and nonviolent person into a murderer.

The most often-prescribed antidepression drug in the world, Prozac has been Lilly’s major moneymaker ever since it was introduced in 1988. Its sales, however, have been falling, for which the company’s annual report blames “changes in wholesaler purchasing patterns and intense competition.” Four years ago, it accounted for more than one-third of Lilly’s total sales. Last year it represented just over one-quarter.

The company’s patents on Prozac will start to expire next year. A patent has been granted for its successor, R-fluoxetine, which Lilly will make and market. In the patent application, which the Boston Globe obtained, the company said the new drug will decrease such side effects such as headaches, anxiety and insomnia, and also “inner restlessness (akathisia), suicidal thoughts and self-mutilation.”

Akathisia, an overwhelming physical and mental restlessness, has been cited by physicians and researchers as a frequent side effect in the cases in which they believe Prozac is causing violent, impulsive and sometimes suicidal behavior.

Teicher, who pioneered research on the question, observed it in a number of Prozac patients who attempted or committed suicide. Teicher is one of the developers of the “new Prozac,” R-fluoxetine, and a co-owner of the patent.
